Use the form below to register for the 2011/12 CDSL singles and doubles competitions. The deadline for entries is January 13th and payment is due by January 31st.
1. Entries must be through this form or by text to Steve Wellbeloved at 07886179583. You will be sent an acknowledgment text or email.
2. If you are writing cheques please make them payable to the Cardiff and District Snooker Association and if you are paying for more than one person either write down the player and comp details on the cheque or include a copy of your entry form.
3. The deadline for entries is January 13th and payment is due by January 31st. Entries for competitions cost £2.50 per person, or £5.00 per doubles pairing. Please send, or drop off, a cheque payable to the Cardiff and District Snooker Association to Steve Wellbeloved at the Mackintosh Sports Club, 38 Keppoch Street, Roath, Cardiff CF24 3JW. Don’t bother sending them to Spence’s old address as he has moved house. Bank transfer is also fine.
4. Once confirmation of all entrants have been posted on sunday the 15th, there will be no late entries allowed.
5. Any person/s who has not paid their entry fees by Jan. 31st will be scratched. If they have already played their first match by Jan 31st and won, and still not paid, they will be scratched owing the CDSA their comp fee.
6. In the event of disputes over matches not played on time, Players who give out contact nos`( ie:- home or mobile ) other than their club no. will have an advantage ( but not guarranteed ) in the decision as to who made the better effort to contact their opponent.
NOTE THAT SUBMISSION OF THE FORM CONSTITUTES ENTRY INTO THE COMPETITION AND YOU WILL BE CHARGED ENTRY FEES FOR ALL THE PLAYERS THAT YOU LIST
