CDSL competition registration 2013/14

Use the form below to register for the 2013/14 CDSL singles and doubles competitions. The deadline for entries is November 20th and payment is due by January 1st. The competitions are this year being administered by the CDSL Secretary Kevin Brown, who can be contacted at onlythestrongestwillsurvive@hotmail.com.

1. Entries must be through the form below or on the appropriate threads on the League Forum at Snookertalk.co.uk.
2. No entries will be accepted without contact telephone numbers. If you do not wish to post your tel number on the forum, please send to Kevin via private message.
3. If you are writing cheques please make them payable to the Cardiff and District Snooker Association and if you are paying for more than one person either write down the player and comp details on the cheque or include a copy of your entry form.
4. The deadline for entries is November 20th and payment is due by January 1st. Entries for competitions cost £3.00 per person, or £6.00 per doubles pairing. Please send, or drop off, a cheque payable to the Cardiff and District Snooker Association to Spencer Symmons at the Mackintosh Sports Club, 38 Keppoch Street, Roath, Cardiff CF24 3JW. Bank transfer is also fine – please contact committee for details.

NOTE THAT SUBMISSION OF THE FORM CONSTITUTES ENTRY INTO THE COMPETITION AND YOU WILL BE CHARGED ENTRY FEES FOR ALL THE PLAYERS THAT YOU LIST
